About the role

To manage a team of a small team support workers delivering an enabling support based semi-independent community living home environments for young people aged 16-18 leaving the care of a local authority and transitioning to adulthood. The successful candidate would need to be committed to providing a high quality safe and supportive living environments for children and young people able to encourage them to live safely in the community. The successful candidate would need to have the skills necessary to work with the Senior Team Leader to train, develop and mentor a team of workers able to enable and empower young people to maximise their potential and transition safely into adulthood.

There’s no such thing as a “typical day” in one of our semi-independent homes and you never know what you might end up doing and they could be supporting someone to make their breakfast in the morning or attending meetings to review their progress in the afternoon. And you’ll be working in a team of skilled, dedicated colleagues who’ll support you all the way.

We need confident and dynamic Team Leaders, who can guide, support and develop a team of support workers to ensure they provide the right advice; support and assistance to young people leaving the care of local authorities aged 16-18. About you

You need to have significant experience of support work including working with young people, leadership skills, finance, rota management, be committed to the values of ‘Every Child Matters’, have experience of Key Performance Indicators (KPI’s) and be educated to NVQ (3) level or above. What matters to us most is your positive attitude to life and your desire to support people to live their life with as much independence, choice, dignity and control as possible.

Our North West based services provide support to individuals in shared ‘Supported Living’ environments across the region as well as individuals in sole placements. Our services seek to offer support services to young care leavers that maximises their independence and life experiences through our teams of dedicated support workers who enable and empower young people to achieve.

Job Description – Team Leader

Department: Operations

Where you will be working: Liverpool/North West

Reports to: Regional Manager, Senior Team Leader

This job is important because:

You will enable the people we support to lead fulfilling lives based upon their individual support needs. You need to work to the standards set internally by Transitional Plus Care (TPC) and externally by the national guidance on the provision of ‘Other Arrangements’ accommodation for care leavers. In doing so, you will ensure that the people we support and your colleagues are supported safely in the community. You will achieve this by effectively communicating with individuals, team members, seniors and managers and by following the agreed Pathway Plan and Supported Living Plans and by:

  • Making sure that you support young people to be as involved as possible in every moment of their lives.
  • Offering young people opportunities to be involved as often as possible, in the way that best suits them, e.g. as little or as much as they like.
  • Offering ‘just enough’ support to ensure young people succeed.
  • Ensuring that the young people you support make as many choices as possible, so they remain in control.

 

Main Duties:

Leadership

  • Oversee a group Support Staff to ensure the delivery of semi-independent living services to young people meets the standards set by TPC.
  • Ensure unit compliance and service delivery is in accordance with TPC standards.
  • Undertake a regular audit role to monitor unit compliance.
  • Provide on call cover as agreed with the Senior Team Leader and Regional Manager.
  • Ensure effective communication.
  • Participate in the formal Recruitment and Training of TPC staff.
  • Liaise with the Senior Team Leader management of staff performance, competency, sickness and discipline.
  • Motivate, support, supervise and mentor Support Workers to ensure continuity of service delivery.
  • Oversee and managing all unit day-to-day operational functions.
  • Ensure clear and effective communication at all times.
  • Ensure all support planning meets the requirements within each young person’s Pathway Plan.
  • Provide effective supervision to support staff.
  • Develop and maintain positive professional relationships at all times.
  • Attend such meetings as required to ensure the service delivery and support for young people provided meets the required needs.
  • Liaise with the Senior Team Leader and Regional Manager on all issues linked to safeguarding and child protection and act ‘First Response Officer’ for all incidents.
  • Provide 24/7 flexible working shift patterns, including weekends and Bank Holidays across multiple sites (if required).
  • Manage unit rotas ensuring adequate staff cover at all times.
  • Provide effective supervision to the Support Workers.
  • Provide ‘on call’ cover as agreed by the Senior Team Leader and Regional Manager.
  • At each shift ensure staff complete all necessary paperwork to an acceptable standard and sign off.
  • Work alongside the Senior Team Leader and Regional Manager to ensure full unit compliance across all areas of the service delivery.
  • Support, implement and manage any planned service changes.

Support

  • Support people with regards to decision making and lifestyle choices enabling them to take control of their own lives, increasing independence.
  • Ensure the support delivered is to the standard set by TPC and in line with their individual support plan, cultural needs and wishes.
  • Support people to take an active role in the community by accessing local facilities and wider community resources in the spirit of social inclusion.
  • Ensure the security of the building and the safety of the people living in the home.
  • Support the management of each individuals Pathway Plan goals are planned, monitored, reviewed and delivered.
  • Support people to develop and expand social networks and friendships.
  • Support people through the transition to adulthood process.
  • Contribute to / attend reviews and multi disciplinary meetings.
  • Manage, monitor, audit and review the delivery of the AQA based Independent Living programme for all young people.
  • Work with individuals to promote healthy life style by liaising with local health care professionals.
  • Chair and manage onsite team meetings and other professional meetings as required by Senior Managers.
  • Work in partnership with TPC managers and other professionals to maximise quality of life for young people.
  • Develop positive relationships with family and carers.
  • Work in line with TPC Safeguarding, Child Protection Policies and Whistle Blowing Policies to ensure that people are kept safe at all times.

  • Ensure risk assessments are completed, monitored, reviewed and followed in line with TPC policies and procedures.
  • Be flexible and responsive at all times to meet the changing needs of the service and the service users.
  • To support the administration of medication in line with the policy and the individual’s Pathway Plan care.

Performance and Development

  • Maintain confidentiality.

  • Adhere to the requirements set out with TPC ‘Other Arrangements’ guidance and staff handbook.
  • At all times work within current legislation.

  • Attend training on a regular basis to ensure that all mandatory and personal training and development needs are met in line with company requirements.
  • To work in accordance with the company’s mobile phone and IT policies. To behave in a manner that reflects positively on the company at all times.
  • Work closely with other members of the team for the ultimate benefit of the people living in the home.

Finance

  • Support people to manage their resources and finances.
  • To ensure that you receive all receipts, change and any accompanying paperwork before the end of the shift.
  • To give practical support to individuals in line with their Pathway Plans and TPC policy and procedures.
  • Ensure Petty Cash and people’s monies are checked, balanced and recorded at the end of your shift.
  • Financial discrepancies must be reported to the Regional Manager immediately. To keep updated with all financial policy and procedures.

Administration

  • You will be required to work flexibly on a rota basis to meet the needs of the service, this includes weekends, bank holidays and waking nights shifts (when necessary).
  • Ensure work is recorded accurately and appropriately in compliance with the company requirements and that records made and personal information used are in compliance with the Data Protection Act and the standards of information governance.
